Genital surgery is a broad term for operative procedures in the genital area, whether it be for aesthetic reasons or because the patient feels discomfort from certain problems, for example in the case of labia reduction.
With a growing acceptance of surgery designed to alter appearance, and the increasing availability of resources, many women are requesting surgery to alter their intimate appearance with a feeling that they compare unfavourably to others. Behind this increased demand is the realisation that size or shape of their vagina or external genitalia prevent them or their partner from achieving sexual satisfaction to fullest extent.
At Saraswat Hospital the most commonly requested such procedures are labial reduction also known as labiaplasty and vaginal tightening operations. Other Aesthetic Surgery procedures include liposuction of the mons pubis; fat injections to the labia majora or mons to give a more youthful appearance to them. Removal or reduction of skin around the clitoris (‘hoodectomy’), to improve clitoral sensitivity; and reconstruction of the hymen for cultural reasons.
Bikini Line Laser Hair Reduction therapy is used for the removal of labial hairs. Hair transplantation is also performed for hypotrichosis (decreased hair growth) of the mons pubis.
Labial reduction is designed to decrease the size and degree of protruberance of the labia minora, or ‘inner lips’ of the vagina. The most common reason for women to request this operation is because the labia are perceived to be too large and aesthetically un-aesthetic. Sometimes large size of their labia make them noticeable in tight clothing, or make sports such as cycling uncomfortable. Some complain in problems with penetration.
The procedure is relatively straightforward, excising an area of the labia minora under local anaesthetic.
Vaginal tightening operations are requested by few women who feel their vagina has become lax following childbirth, or naturally with age. Consequently, they percieve that sex is less satisfying for them or their partner. The procedure involves excising excess vaginal mucosa alongwith tightening of muscles.
Liposuction or ‘liposculpture’ of the mons pubis may be an isolated procedure or performed in conjunction with liposuction of the abdomen or thighs. This improves the definition of Mons.
Sometimes fat injections to the mons or labia majora is needed to make them plump giving them a more youthful appearance.
Hoodectomy means removal of the fold of skin around the clitoris, this is performed to expose the clitoris and make it more sensitive.
The surgical procedure consists in tightening the vaginal opening and thickening of the vaginal mucosal walls. This original method concerns in most cases multiparous women but also young nulliparous women presenting with wide vagina without perineal lesions. The procedure is ambulatory, under local or local regional anesthesia.
Procedure is started with tightening of the muscles in the posterior perineum followed by a lipograft in the lateral walls of the vagina. The vaginoplasty technique consists in tightening the levator muscles of the anus. The muscles are individualized and joined by suturing.
The complications are rare, corresponding to any other surgical procedure in this very area and have not jeopardized the final result.
Fat is extracted from the pubic area, the internal and external thigh area, and sometimes the buttocks. These areas are rich in adipose tissue with alpha2 receptors that are insensitive to variations in diet and weight as opposed to adipose tissue with beta1 receptors. This results in better retention of the graft over time. 50to 70 mL fat is extracted using 2 mm diameter foam tip catheter (mounted on a 20 mL syringe).
