How Laser Tattoo Removal Works?
In tattoo removal, the target for the laser light consists of small particles of tattoo ink which are found either within macrophages or scattered extracellularly throughout the dermis.
Q-switched lasers are the mainstay of therapy for both benign pigmented lesions and tattoos.
Why Tattoo Removal?
The recent growing trend of decorative tattooing among teens and young adults has led to an increase in the number of patients requesting tattoo removal. Most tattoos encountered today are professional tattoos which are more difficult to remove than the amateur variety, due to the fact that they are often composed of multiple colors of inks placed at a variety of depths within the dermis that are often impossible to remove completely with the current technology. Different colours of tattoos require different sources of lasers for removal which practically is very difficult to find in one medical facility.
Q-switched Frequency-doubled Nd:Yag: 532 nm. Very nicely absorbed by melanin so effective for age spot or sun spot removal. Somewhat helpful in red and orange tattoos.
Q-switched Ruby: 694 nm. Best wavelength for blue ink.
Q-switched Alexandrite: 755 nm. Works well on green tattoos
Q-switched Nd:YAG: 1064 nm. For black tattoos and the only laser suitable for darker skin.
Thus establishing realistic expectations for every patient is paramount in achieving a result with which the patient will be satisfied.
How Many Sittings are Required?
It must be emphasized that multiple treatments are normally required and may vary anywhere from five to as many as10 in number. Furthermore even after numerous treatments, some tattoo pigment may still remain.
Cosmetic Tattoo Removal
When tattoos are used to enhance the shape of the upper or lower lips, reconstruct or augment the appearance of the eyebrow or to reconstruct the appearance of the eyebrow or to reconstruct the appearance of the areola following mastectomy, the technique is known as cosmetic tattooing. The tattoos ink are usually a mixture of white(titanium) and red (ferric oxide) pigments, the relative concentration of each is determined by the location and the natural pigmentation of the affected area. On occasion these tattoos are applied inexpertly , or the cosmetic style has changed making the appearance of the tattoo look ‘old fashioned’ or obsolete or the patient may just not like the appearance. In such cases patient request for tattoo removal which is done by Q-switched laser.
